Case Management

Brookhaven Memorial Hospital provides patients with a Case Manager. Case Managers work with doctors and other clinical staff to help patients and their families make decisions about their stay at the hospital, and work with community resources to arrange for continued care after discharge. Some of these resources include: home care, nursing homes, caregiver support groups, and community resources. Case Managers may also keep in touch with a patient's insurance company and give them the information they need to provide coverage. The Case Management Department has staff that can assist you in obtaining services after you leave the hospital. They can meet with you and your family to discuss the services that would best meet your needs. Case Managers have training in assisting with a broad range of individual and family concerns. They can also provide referrals to agencies in the community. 
There are many support groups that meet at the hospital. You can contact Patient Education at 631-654-7168 for information about a specific group. Your Case Manager can assist you in finding the right group or community resource that will meet your needs. 
The Case Management Department's team of dedicated nursing and social work professionals work in close collaboration with physicians and other health team members to help patients and families with the problems that can arise as a result of illness and hospitalization. In addition to assistance with developing an individualized discharge plan, the department coordinates links between patients, family support systems and community resources to enhance the patient's optimum functioning in the community. The Case Management Department serves all patients from newborns to geriatrics, including all inpatient services, pre-admission testing, day surgery, out-patient chemotherapy and emergency services. 
Model of the Future
Case Management uses a model, which integrates Utilization Management, Social Work, Continuing Care and Clinical Guidelines. The Case Management Department retains responsibility for discharge planning and counseling interventions. Case Managers are either Nurses or Social Workers. They work as teams with core assignments and are cross-trained to learn a broad range of functions in the department.
You may also work with a nursing home/sub-acute Placement Coordinator and Case Assistants who are part of the Case Management team. A Medical Director who is an M.D. and the Department's Management, Team that follows the Departments Social Worker and Nurse team model, provide leadership.

Hours of Operation
The department's operating hours are 7:00 a.m. - 5:00 p.m., Monday - Friday. Voicemail operates after hours. Limited Case Management staff are also available on Saturdays from 8:00 a.m. - 4:00 p.m. to assist with home care arrangements for discharged patients. The Emergency Department is covered 8:00 a.m. - 11:00 p.m., 5 days a week.
