Questions and Concerns About Your Care
Being in a hospital can be unsettling. Sometimes, you may have a question and not know who to ask. The first person you will want to ask is your physician or your nurse. Your concerns and issues are of great importance to us.
Unresolved issues may be addressed in the following manner:
- Ask to speak with the Nurse Manager
- Ask to speak with the Patient Advocate (Monday - Friday, 8 a.m. - 4 p.m.) by dialing 631-687-4655
- During all other hours, ask to speak with the Nursing Supervisor by dialing 631-654-7109
Who Can Contact the Patient Advocate
Any patient at Brookhaven Memorial Hospital Medical Center or any family member may contact the Patient Advocate. In addition, the Patient Advocate may receive a referral from any member of the case management staff, physicians, nursing, chaplains or other staff members.
What Will the Patient Advocate Do For Me?
Once the exact nature of your concern is clear, the Patient Advocate will assist in obtaining answers to your questions and/or assist in reaching a mutually acceptable solution.

Unresolved Concerns/Problems/Complaints
If you have a concern, problem or complaint related to any aspect of your care during your hospital stay, speak to your patient advocate, doctor, nurse or hospital staff member. If the hospital staff has not resolved the problem, you may contact the New York State Department of Health by phone or by mail.
