Vial of Life
The Vial of Life is designed to speak for you when you can't speak for yourself. The "vial" contains important medical information that can assist emergency personnel in administering the proper medical treatment.
The components of The Vial of Life are:
- 2 Vial of Life stickers
- Vial of Life form
- Plastic Baggie
Vial of Life Steps:
- Fill out the Vial of Life Form
- Place completed form in plastic baggie
- Attach plastic baggie to front of refrigerator
- Place Vial of Life Sticker on front door and refrigerator
If emergency personnel are called to your home the Vial of Life Sticker on the front door alerts them to look on your refrigerator for the baggie containing your medical information. This information can then be shared with medical personnel to assist in your care.
