Brookhaven Memorial Hospital's Coumadin® Management Center is here to provide you with the careful monitoring you deserve while on anticoagulation therapy. The Coumadin® Management Center is an outpatient service of Brookhaven Memorial Hospital Medical Center which was established for the purpose of managing and ensuring high quality care to patients who are taking Coumadin®.

Many individuals use Coumadin® or warfarin therapy for the treatment or prevention of blood clot formation. The Coumadin® Management Center at Brookhaven Memorial Hospital Medical Center is now available to provide a comprehensive approach to patients on anticoagulation therapy. This multidisciplinary program includes the services of a physician, nurse practitioner, pharmacist and registered dietitian. At the Coumadin® Management Center the blood test used to guide management (known as an INR) is done by a simple fingerstick and the results are immediately available. Coumadin® dosage changes are made at the time of the blood test if indicated. Additionally, patients receive education about Coumadin® therapy including: medications interaction, dietary and safety issues.

Oral anticoagulation with Coumadin® is the drug of choice for the treatment or prevention of many blood clotting disorders including deep vein thrombosis, pulmonary emboli, mechanical heart valves, embolic stroke, and atrial fibrillation. Coumadin® therapy can prevent devastating events related to blood clots, such as stroke, but must be monitored closely. Coumadin® therapy is guided by a blood test known as PT/INR (Prothrombin Time /International Normalized Ratio). The PT/INR can be affected by numerous factors including: dietary intake of Vitamin K, other vitamins and herbal supplements, other medications including prescription medications, over-the-counter medications, alcohol consumption, and other illnesses that affect the metabolism of Coumadin®.
