Certified Home Health Care
Certified Home Health Care offers a coordinated plan of care and services, under the supervision of the physician, to patients who might otherwise require continued hospitalization or other institutional care. Makes it possible for patients to continue their recuperation at home with the availability of services provided by skilled professional. Is also available to individuals who have not been recently hospitalized if they still meet the eligibility criteria.
Certified Home Health Care services available may include:
- Skilled Nursing on an Intermittent Basis
- Medical Social Services
- Physical Therapy
- Speech Therapy
- Certified Home Health Aides (Only in Conjunction with Skilled Care)
- Medical/Surgical Supplies and Equipment
- Laboratory Tests
- X-Rays
- Ambulance/Ambulette To the Hospital
- Prescription Medications
- 24-Hour Availability of Professional Staff
Eligibility for Certified Home Health Care
The patient must be residing in Suffolk County when the services are provided. The patient must be essentially homebound and have needs that can be managed within the scope of the services offered. The patient's physician must continue to provide medical supervision during the time the services are provided. The patient's residence must be suitable for appropriate, professional health management at home.
Long Term Home Health Care
The Long Term Home Health Care Program offers a coordinated plan of care and services to the disabled, infirmed, or invalid person who would otherwise require placement in a skilled or health related nursing facility. This program makes it possible for the chronically ill patient to remain in his or her own home.

- Physical/Occupational/Respiratory Therapies
- Speech Therapy and Audiology Services
- Laboratory Services
- Nutritional Counseling
- Medical Social Services
- Medical Supplies, Equipment, Appliances
- Transportation
- Case Management
- AIDS/HIV Home Care Program
In addition, the following special services may be provided when necessary:
- Home Improvement
- Social Day Care
- Respite Care
- Congregate/Home Delivered Meals
- Lifeline
Eligibility for Long Term Home Health Care
Eligibility for Long Term Home Health Care will be determined by a number of factors including: a nursing assessment showing that residential health care is required, your physician's order authorizing home care, a coordinated plan of care that remains within 75% of the local nursing home costs, a family member or significant other who is willing to participate in caring for the patient, the patient is Medicaid eligible for Long Term Home Health Care, or has the ability to pay privately, the approval by the Department of Social Services if payment will be made through Medicaid.
