The Telehealth Program

Brookhaven Memorial Hospital's Home Health Agency now offers technology which allows Home Care nurses to monitor vital signs, as well as see and speak with patients from a distance, greatly improving efficiency and continuity of care. The Telehealth program provides a direct computer link between homebound patients with chronic obstructive pulmonary disease (COPD) and nurses at the Medical Center's Home Health Agency in Patchogue.

Under this program, Brookhaven's Home health Agency staff will continue to visit patients in their homes with the added benefit of enhanced monitoring via home Telehealth in between on-site, home visits. This added ability to monitor patients in-between visits allows us to reinforce the education process and to maintain closer contact. This program offers the patient and the family feelings of security, independence and an increased understanding of the patient's health and care.

This new technology allows us to provide:

  • Better access to care
  • Earlier detection and intervention
  • Reduced hospitalizations and emergency room visits
  • Prolonged independence at home
  • Enhanced patient quality of life
  This program has been so successful that The New York State Department of Health recently announced that Brookhaven Memorial Hospital Medical Center's Home Health Agency has been awarded a second grant from the New York State Department of Health in the amount of $150,000. Thomas Ockers, President and CEO of Brookhaven Memorial Hospital Medical Center said, "The grant will allow our Home Health Agency to expand the number of monitors from 19 to 40 and to offer the program to patients with cardiac disease, greatly expanding its usefulness."

The program has already demonstrated improved outcomes, high levels of patient satisfaction, and decreased hospital admissions. Patients feel increasingly secure and are generally comfortable with the technology.

Home Telehealth is most appropriately used for vital sign measuring and the monitoring of other complex medical problems. Patients who are referred and meet the selection criteria will be provided Telehealth at no cost.
